Implementation in each country, a pharmacist-led Medicines Review under different names (Medicine Therapy Management, Medicine Use Review), is a contribution to ensuring patient safety by reducing medication harm. We would like to know, opinion about the perspective of the service implementation in each of the countries globally, as well as barriers identified during this process. Questions, include, description of the service implementation, as well as education that needs to be adapted for optimal and cost-efficient implementation. Sharing opinions among many countries where service is still running, or failed, are very important for us, due to the need of maximizing the service performance and outcome in Poland.
